Leverage and "Notional volume"

I am opening a new thread, because I want to clarify a terminology.

Given LCG's leverage info: https://www.lcg.com/leverage-information/
I'll take into account the indices section, with Margin Group: top Level.

Here, we have 4 tiers:
Tier 1. Notional USD: 1.000.000 / Leverage: 500
Tier 2. Notional USD: 3.000.000 / Leverage: 200
Tier 3. Notional USD: 6.000.000 / Leverage: 33
Tier 4. Notional USD: >10.000.000 / Leverage: 20

At the bottom of the page, we notice: LCG trader tiers are calculated on notional USD volume traded where as MT4 are calculated based on open lots.
Also, we find: As the volume per Instrument of a client increases, the maximum leverage offered decreases accordingly

What does this "notional USD volume traded" means? The all time notional USD volume (for that instrument), or just the current volume traded?

For example:
1. Let's say, at moment X, we "buy" 1 lot of DJIA, at 26000. Notional volume is: 1 * 10$ * 26000 = 260.000 USD.
2. At moment Y, we "buy" 4 lots of DJIA, (at 26000). Notional volumea is: 4*10* 26000 = 1.040.000 USD (which should move on tier 2).

Now, every trade I make is subjected to Tier 2 of Leverage or just the 2nd move is subjected? Just active positions are taken into account when they are calculating your max. leverage or the "all time" notional USD volume/instrument ?

Also, As the volume per Instrument of a client increases, the maximum leverage offered decreases accordingly ... is applicable for the "all time" transactions, or, again, just for the current trade?

current volume traded / open trades

the trades here are on the same symbol DJIA so the tier applies to both
