You have to set the indicator's "Calculate" property to "On price change" or "On each tick" otherwise it will only calculate on a bar's close.

NinjaTrader coupled with CQG is for futures. If you select the forex trial, you will get logon credentials to connect NinjaTrader to FXCM's rate feed. Not sure about ForexTester but the NinjaTrader/FXCM combo is free and unlimited.

If I understand you correctly, that's a question of margin. I believe most competitive brokers require that you put up $500 margin to trade ES.
